You are probably using a softsynth with Noteworthy. Most softsynths have latency to some degree or another - a way around this MIGHT be to use ASIO, but ASIO doesn't like to share the MIDI and you might have some problems even getting input.
For information relating to getting ASIO happening have a look at my "dummies guide" on the Scripto:
http://nwc-scriptorium.org/helpful.html#Vst
and look for "VST for dummies like me" - it has a section on ASIO that might be helpful
If you have a sound card that uses a hardware synth (like the Creative series of cards) then for the duration of input from the keyboard ditch your softsynth and use the hardware one.
Actually, even the standard Microsoft softsynth (Microsoft GS Wavetable SW Synth) is pretty efficient and may have little enough latency for you to get away with it. That said, the hardware synth is far and away the best method.
Another possibility is to use an external synth that will accept MIDI from both your PC AND the keyboard - IE tell NWC to use the MIDI port for output (you may need to purchase a USB to MIDI converter for this...) and set your MIDI interface to do passthrough.
Hope this helps more than it confuses...
