CutePDF Writer (version 3.2.0.1) works for me in Windows 10, but you sometimes have to wait a while for the box to appear.

You can (sort of) do this already if all you want is a printed sheet.

In the staff setup (can't remember the exact words, as I haven't got NWC to hand), you can specify the number of lines to show. Then you can add the fret numbers as text at the appropriate positions.

It's not the prettiest, but I have used it a lot for bass 'tab'. I tend to have a single line stave too, with the rhythm on, and place the fingering text on that stave - positioned to show on the necessary string on the 4 'string' stave.

For me, as a non-expert, it looks like the measure is divided into 4 equal parts - at the beginning you have a 1/4 rest followed by a 1/8 rest, rather than a dotted 1/4 rest.

Because of this, I would tend to go with the second (tied 1/8 notes) option, rather than the first.
