I have recently found a book which answers most of the question about the placement of notation symbols, tempo and other textual elements, dynamics, slurs, ties, style, etc. It is the "Essential Dictionary of Music Notation" published by Alfred Publishing Co.

Rules of course are made to be broken, but while the characterization and plot are weak, this book is very helpful.

Rules are made to be followed!
It is okay to break them on occasion,
but the reason they are made is to be followed.
They are not made just so that they can be broken!
